How do I borrow the
equipment?
Before a district or agency can borrow
a piece of equipment, they must complete an agreement form
which needs to be signed by an authorized district or agency
representative. The agreement form must be sent the
Rockhurst University address listed below and be approved
prior to borrowing equipment.
Once your agreement form has been
approved, you can proceed with your equipment request. Any
of the listed distribution sites can process an equipment
request for up to three items. There is no limit to the
number of loans. If an item is on loan you will be placed on
a waiting list and the equipment will be shipped as soon as
it becomes available.
Items will be shipped via Federal
Express to the district or agency's Federal Express address.
The ETC Program pays all shipping costs. The
loan package will include the equipment with instruction
manual information, return packing slip, and a letter
explaining what equipment you have borrowed and the due
date. In some cases, if you are located close to one of the
distribution sites, you may be able to pick up and return
the equipment rather than having it shipped.
